British Columbia - Notary/Acknowledgment Form

In Texas, when we prepare documents to be notarized, we either prepare them to (a) be signed under oath (a jurat), or (b) have the notary confirm the person appeared in person and said he signed it (acknowledgment of non-forgery). I am dealing with the latter - I need to have a BC resident acknowledge his signature before a notary.

For example, in Texas, the form looks like:

State of Texas §
County of Harris §

This document was acknowledged before me by John Doe on January 1, 2018.


Does anyone have a proven form for use in British Columbia?
