Speeding ticket in NY - do I need client to sign an affidavit of representation?

so the "client" here is my cousin. I don't normally due traffic court but I'm helping her out. This jurisdiction requires in court pleas. I was told before that the attorney needs to have the client sign some sort of affidavit of representation. Can someone clue me in on what form exactly? Thanks.

Yes, even if the particular court doesn't require a "Letter of Engagement," or "Affidavit of Representation," you should have one in your file.

Usually when you enter the plea, the judge will ask "Do you have authority counselor," and of course you answer in the affirmative. Usually, you don't need to provide the court anything else. Some court require a written and notarized affidavit, for instance Village of Huntington Bay requires a very specific form, yet other courts such as Nassau TPVA or SCTPVA could care less.
